The family care guide

A clearer next step.
For the people who care.

Practical planning notes to help your family ask useful questions and feel more prepared when arranging home care.

01 / GETTING STARTED

Before your first care conversation

Write down the support your family is looking for: help with daily routines, nursing visits, recovery support or equipment. Include your area, preferred dates and who the team should contact.

You can start with “I’m not sure what we need.” The first conversation is a place to work that out.

02 / COMING HOME

Planning around a hospital discharge

Ask the treating team for discharge instructions and which follow-up services should be arranged. Share the prescribed requirements with the care coordinator through the agreed communication channel.

Confirm who will coordinate appointments and keep the family informed.

03 / EVERYDAY SUPPORT

Choosing help for an older parent

Talk with your parent about their preferences, privacy, language and familiar routines. Make a list of tasks where support would help, and discuss the difference between a caretaker and clinical nursing.

Involve your parent in decisions wherever possible.

04 / BEFORE YOU CONFIRM

Questions to ask about a care plan

Ask what is included, the visit or shift duration, the professional’s role, the full price, and what happens if plans change. Discuss your point of contact and how concerns can be raised.

Keep a written record of the arrangements you agree on.

A little support. A world of difference.
